Baggage

One carry on and one checked only. Baggage cannot exceed 62 inches. Height + Length + Width and has a 70 lb limit.

You will be taking possibly your horn, drum or or equipment as addition luggage or carry on. Sleeping bags either will be boxed or we will use other sleeping arrangements in Europe.

Please type or print legibly when completing all sections of this application and do not sign the application until you take the oath administered by a passport acceptance agent.

PLEASE PRINT OUT AND SUBMIT ONLY ONE-SIDED PAGES OF THE APPLICATION.

You must use this form if:

You have never had a U.S. passport
You are under age 16
Your most recent passport was issued more than 15 years ago
Your most recent passport was lost or stolen. (If you are applying because your most recent, valid passport was lost or stolen, you will also need to submit Form DS-64 listed on the left.)

Please follow instructions on pages 1 and 2 and read carefully pages 3 and 4. Complete both pages of the application and print on one-sided pages (do not sign the application). Bring pages 3 and 4 of the instructions along with the application when you appear at the acceptance facility or passport agency to apply for your passport.
